Comox Valley Nursing Centre (CVNC) - Chronic Disease and Chronic Pain Program

The Chronic Disease and Chronic Pain programs offers individual and group support to assist those living with complex chronic conditions. We work as a team to provide education, self-management support, links to resources and connection to various other programs.
The Chronic Disease and Chronic Pain program offers individual and group support to assist those living with complex chronic conditions. A team of registered nurses, physiotherapists and social workers' provide education, support groups, self-management support, relaxation therapy, links to resources and connection to various other programs. The team will help patients address barriers to wellness and develop an individualized self-management plan.

To Access This Program:
  • Request a referral from a primary care provider
  • Those that don’t have a primary care provider may self-refer by contacting the centre at: 250-331-8502

Hours: Monday to Friday: 8:30 a.m. to 6 p.m. Closed 12 p.m. - 1 p.m. Closed weekends and statutory holidays. Note: Patients may drop-in or call for inquiries.
